The Municipal Civil Registrar (MCR) Office of the Local Government Unit of Santo Tomas, Isabela invites residents to avail of the FREE Request for PSA Copies of Certificate of Live Birth, Marriage Certificate, Death Certificate, and CENOMAR on July 1, 2026, from 8:00 AM to 5:00 PM at the Municipal Hall, Santo Tomas, Isabela.

Requirements:
• Valid ID / National ID

For Representatives:
• Authorization Letter
• Valid ID of the document owner
• Valid ID of the representative

FREE for the first 200 clients only (one request per client).
